Custom Form Submissions are the completed/answered Custom Forms submitted by users. Custom Form Submissions can be accessed by choosing Activity () and then Form Submissions from the menu.

Understanding the Form Submissions page
The Custom Form Submissions page is used for managing submitted forms.
The page has the following features:
- Navigation button to Manage Forms for existing ('blank' / template) Custom Forms.
- Filters for the list.
A single filter or a combination of filters can be used. The Apply Filter(s) button will set the chosen/entered filters on the list, whereas the Reset Filter(s)button will remove any current filters and restore the full list.- Form ID - Finds a specific form with the entered ID (if it exists).
- Form Type - limits the list to include only submissions of a particular form.
- Date Range - limits the list to include forms submitted during the specified date range.
- Name - limits the list to only forms submitted with the specified name (can be either given name or surname).
- Email - limits the list to include only forms submitted with the specified email address. It is not case-sensitive, but the entry must match the email supplied for the form in full, part words or domains only cannot be used.
- The Display archived submission checkbox toggles the inclusion of archived records in the list.
- The list of submitted forms.
- ID link - click a form's ID to view the completed form. This will open the Custom Form Submission page to show the content of the completed form.
- assign - choose a VETtrak Client record to assign the form.
- export - preview a print-friendly version of the completed form in a new browser tab.
- archive - remove the record of the completed form from the list.
Viewing a Form Submission
More details for each of the records in the list of submitted forms are available if the individual submission is viewed.
- Click the ID link ( with a number) for the form.
This will open the submission:
The Custom Form Submission page shows the submission as it was completed by the user on submission, however, it also features the page heading and some buttons:
- Back — returns to the Form Submissions page.
- Export PDF — downloads a pdf of the current submission.
- Submit — re-submit the form with change(s). This may be necessary if your form includes fields intended for completion by staff members, such as an Office Use Only section.
